We have got a certified team of medical professionals offer community nursing care solution under the NDIS scheme. Each participant who’ve got community nursing care in their NDIS plan is eligible for our service. They’ll be provided with a medical staff who’ll be in constant touch to monitor, discuss and provide relevant feedback to the participant’s medication, treatment and progress.
The service includes:
- Nurse visits and assessments
- Preparing development plans
- Case management
- Medication setup
- Medication supervision or administration
- Medical support like wound care, stoma care, respiratory support, diabetes management, dementia management, monitoring of vitals etc.
- Overnight nursing care
- Education for participants and their family
